Welcome to Centennial Youth Lacrosse

 Please click HEREto read or download the CYLA Registration and Informational Packet. It is important to note that there have been several changes this year. It is your responsibility to make sure you understand the information provided. Board Members will be available at registration to answer any questions.

The following items 1 – 3 are REQUIRED at the time of registration

NOTE: All forms are also located on the CYLA FORMS page, please print, complete and sign all forms to register

1) PRINTED COPY of US Lacrosse Registration confirmation for 2012 season.

  • NOTE: USL membership MUST be renewed prior to registration even if not yet expired.

2) 2012 CYLA Player Registration Form and payment.

  • Click HERE to read Parent Code of Conduct


3) Volunteer Form AND volunteer deposit.

  • NOTE: Volunteer check is separate from payment check
