The following items 1 – 3 are REQUIRED to register for lacrosse:

1) US Lacrosse Registration confirmation for 2010 season (go to the link Looking for Your Member Number at the top of the page, then click the link in#1 go to Member Lookup and print USL number and expiration date). USL membership MUST be renewed at time of registration even if not due until later in the season. Click HERE to access their website.
2) 2010 CYLA Player Registration Form and payment
3) Volunteer Form AND volunteer deposit. (Volunteer check is separate from payment check!)
